HARBOR BEACH NURSING AND REHABILITATION CENTER ACHIEVES 5-STAR QUALITY MEASURES RATING

Ft. Lauderdale, Florida (May 09, 2016) – Centers for Medicare and Medicaid Services (CMS) created the Five-Star Quality Rating System to help families and caregivers compare post-acute providers more easily. CMS uses an overall scale of one to five stars, and a separate rating for each of the following three sources of information: Health Inspection ratings that contain information from the last three years of onsite inspections, including standard surveys and complaint surveys; Staffing which is rated on the number of hours of care on average provided every day, by nursing staff, to each resident and their level of need; and lastly, the Quality Measures rating which evaluates information on eleven different physical and clinical measures for patients and residents. Quality Measures ratings are based on resident assessment data that show how well the nursing home cares for residents’ needs.

As a part of the Consulate Health Care family, Harbor Beach Nursing and Rehabilitation Center, a 59-bed skilled nursing center located in Ft. Lauderdale, FL, has achieved five stars in Quality Measures and an overall 4-star CMS rating. When asked about the morale of the care center, Director of Clinical Services, Mercedes Wynruit stated, “Our focus on resident-centered care has really empowered all staff to look at each resident individually, developing an appropriate individualized care plan and pairing that resident with the right caregiver to achieve the best clinical outcomes. This philosophy has really transpired throughout our center, which we can witness from the residents.”

Executive Director, Frank Phillips, spoke to the processes and organization of his care center, ensuring that workflows, supplies and activities are exactly as they need to be to promote efficiency. “Making sure that the nursing cart is appropriately placed with the necessary medications is a small task that can have a big, lasting impact. Saving that nurse’s time from trying to find her supplies allows her to care for her residents more efficiently,” said Phillips. “We also have an open-door policy for staff, residents, families and even visitors. If someone comes in for a tour in the evening, we accommodate them. We greet residents as we make our rounds, and we encourage open communication. This interaction promotes teamwork, all the way through nursing, housekeeping and dietary.”

The continued focus on accountability, working towards safe clinical outcomes, all while encouraging staff engagement, has helped Harbor Beach attain an improved overall CMS rating. “Keeping employees accountable for their actions not only creates a more productive work environment, but it also creates a positive patient experience, resulting in individualized care with positive outcomes,” said Phillips. “We use Consulate’s Core Value Program to encourage staff to continue excelling in their role and strive to become better leaders. We’re family here, we want the best for everyone.”

For Consulate Health Care, a national leading provider of senior healthcare services and the largest post-acute provider in Florida, this Quality Measures rating is evidence of a commitment to providing the best possible care to those they serve on a daily basis. Consulate’s employees strive every day to make a positive difference in the lives of those entrusted to their care. This national recognition proves their dedication to caring for patients and residents like family, while living out their mission of “Providing Service with Our Hearts and Hands”.
