Shoal Creek Rehabilitation Center Earns 2016 Silver

National Quality Award

— Shoal Creek Rehabilitation Center, a Consulate Health Care center, recognized by national program for commitment to care —

(Crestview, FL) – Shoal Creek Rehabilitation Center has earned the 2016 Silver – Achievement in Quality Award by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). The award is the second of three distinctions possible through the AHCA/NCAL National Quality Award Program, which recognizes providers across the nation that have demonstrated their commitment to improving quality of care for residents and patients in long term and post-acute care centers and communities.

Consulate Health Care is a leading national provider of senior healthcare services, specializing in post-acute care, offering services ranging from comprehensive short-term rehabilitation and transitional care, to Alzheimer’s and dementia care. As a leader in short-term rehabilitative programs and transitional care plans, Consulate is honored to add another Silver recipient to their previous list of six Silver and 120 Bronze Award recipients.

“I feel extremely honored receiving the AHCA Silver Award. Shoal Creek is an awesome facility and we work very hard; it is a great feeling for our team to receive this well-deserved recognition,” said Deborah Montenaro, Executive Director of Shoal Creek Rehabilitation Center. “Last year the Florida Governor’s Gold Seal Award, this year the AHCA Silver Award. Shoal Creek will be working towards the [AHCA] Gold.”

Based on the core values and criteria of the Baldrige Performance Excellence Program, the AHCA/NCAL National Quality Award Program challenges member providers to achieve performance excellence through three progressive levels—Bronze, Silver, and Gold. At the Silver level, members develop and demonstrate effective approaches that help improve performance and health care outcomes.

“It takes incredible dedication and passion to successfully apply the demands of the criteria,” said Chair of the AHCA/NCAL National Quality Award Board of Overseers Christine Boldt. “I applaud Shoal Creek Rehabilitation Center for reaching this level and for its commitment to always improve.”

As a recipient of this year’s award, Shoal Creek Rehabilitation Center can now advance in developing approaches that meet the criteria required for the Gold – Excellence in Quality Award.

The awards will be presented to honorees on October 18th during AHCA/NCAL’s 67th Annual Convention & Expo in Nashville, Tennessee.
